SSI/SSDI Outreach, Access and Recovery (SOAR)

SSI/SSDI Outreach, Access, and Recovery (SOAR) is designed to increase access to the disability income benefit programs administered by the Social Security Administration (SSA) for eligible adults who are homeless or at risk of homelessness and have a mental illness and/or a co-occurring substance use disorder.

Many adults with co-occurring disorders who are homeless have trouble completing the SSI/SSDI application process and, consequently, have no consistent source of medical care. SSI/SSDI Outreach, Access, and Recovery (SOAR) can help. SOAR is designed to improve access to Social Security Income (SSI) and Social Security Disability Insurance (SSDI) for people who are homeless with disabling conditions such as co-occurring disorders. Individuals can use the cash assistance from SSI/SSDI to access housing as well as medical and behavioral health services through Medicaid.

The program serves homeless individuals in the following counties: Antelope, Burt, Cedar, Cuming, Dakota, Dixon, Dodge, Knox, Madison, Pierce, Stanton, Thurston, Washington and Wayne counties
Eligibility & Application:

Step One

Confirm Eligibility

This program is for individuals who are experiencing homelessness and have a serious mental illness.

If you or someone you know are experiencing 3 out of the 5 conditions, please go to Step Two:

  • Are you homeless or at risk of homelessness?
  • Do you have a mental health diagnosis?
  • Are you currently unemployed?
  • Do you have a history of hospitalization and/or medical care?
  • Do you still need to start your application?

Step Two

Refer Client Click on SOAR Referral

Step Three

When a referral is received, the SOAR coordinator will confirm eligibility and follow-up with the client and the referring agency.

If client is eligible we will need a Consent Form and Release of Information completed.  Click on the Consent Form and Release of Information Form to complete the forms.
