Welcome to the Affordable Care Act Navigator Assistance program home page!

Navigators help people understand the Affordable Care Act, complete eligibility applications and enroll in health insurance through the Health Insurance Marketplace or “the Marketplace.” We also assist anyone who needs an exemption from the fine for not having health insurance and with appealing a Marketplace eligibility decision. Medicaid and CHIP applications are also a part of Navigator services. Navigators are completely neutral and unbiased. We are not insurance agents affiliated with any health insurance company, nor do we influence your decision to pick one company over another. Navigator services are available to everyone and are free of charge. You can access our privacy statement here. Thinking about completing an application to see if you’re eligible? The following is information you’ll need for anyone applying:

  • Home and/or mailing address
  • Date of Birth and Social Security Number
  • Documentation for legal immigrants
  • Tax documents for the most recent year
  • Income (examples: copies of pay stubs or W-2 forms, bank statements, letter from Social Security stating income, etc.)
    • Include your best estimate of 2016 household income
  • Completed Employer Coverage Tool (find it here:
  • Current coverage documentation (insurance card, summary of benefits, plan brochure, etc.)

Did you have a life change within the last 30 days? Navigator assistance is available year-round! We can help you report the change to the Marketplace. Here’s a brief list of life changes to report:

  • Marriage status
  • Household size or pregnancy
  • Increase or decrease in household income
  • Getting coverage through a job
  • Turning 26 (no longer eligible for coverage under a parent’s health plan)
  • Becoming eligible for Medicare or Medicaid
  • Moving (any distance)
  • Tax filing status and changes to your name, date of birth or Social Security Number
  • Disability status
  • Citizenship or immigration status
  • Incarceration status
  • American Indian/Alaska Native tribal status

NOTE: Some changes don’t affect your coverage or savings but still need reported.

Navigator Privacy Notice

Data Collection and tracking

This Agency receives funding from the Centers for Medicare and Medicaid Services (CMS) to provide enrollment assistance to Nebraskans using the health insurance marketplace. This funding gives Community Action Agencies in Nebraska legal authority to collect and track personally identifiable information (PII) of the people we assist. The purpose of the information collected is to help document the use of the Affordable Care Act Marketplace and report the number and characteristics of those assisted to our funder. The information we collect from you will be disclosed only to the grantee, Community Action of Nebraska, and this agency, and only for the purposes of reporting as required by law. None of your PII will be shared outside Community Action and all reporting completed will include aggregate data only, so none of your personal information will be shared with anyone other than the agency with which you provide it and their grantee, Community Action of Nebraska, who will use it for reporting purposes only. Refusal to provide information will NOT constitute a denial of services.  All personal information collected from you will be destroyed no later than 10/2/2017.

We only collect information that we consider to be appropriate. The collection and use of all personal information is guided by strict standards of confidentiality. A copy of our Privacy Notice describing our privacy practice is available to all consumers upon request.
