Immunization Program

Attention all: there is some questions popping up regarding the new Immunization regulations that the CDC and Federal Government will be implementing on October 1st, 2012. Northeast Nebraska Community Action Partnership will be affected by this change. We will only be able to give Immunizations to those children who have no health insurance, who are utilizing Medicaid, those who are American Indian/Alaska Native or those under-insured. Under-insured means that a person may have health insurance but the coverage does not include immunizations. Please contact Northeast Nebraska Community Action Partnership with questions or concerns.

The following forms are paperwork you can have completed and printed off before you get your immunizations at one of our clinics.  You will need the free version of Adobe Reader on you computer to view these forms.  Click Here if you do not have the free version of Adobe Reader downloaded on your computer.
