IMMUNIZATION REQUIREMENTS

There must be a parent or guardian accompanying the child. If someone else brings the child to clinic, must have a proxy form and health questionnaire filled out and signed by parent. You may call the Pender office at 402-385-6300 to obtain the proper forms. There is suggested donation of $19.50 for a single vaccination, or a suggested donation of $39.00 if receiving 2 or more. No one will be denied vaccinations for inability to pay.

*Immunization records are required at each clinic visit.*

It is important to follow up with your family physician for well-baby or childhood checkups on a regular schedule.
